coronary heart disease, chronic obstructive pulmonary disease, diabetes and chronic kidney disease.

Many illnesses and health conditions can be classified under the broad heading of chronic disease.
Chronic diseases are mostly characterized by:
-Complex causality
-Multiple risk factors
-Long latency periods
-A prolonged course of illness
-Functional impairment or disability

Patients who have a chronic condition are entitled to a health care plan and reviews each year. Our doctors and nurses will help you set
health goals and discuss how these can be met. These plans are used by our team to monitor and manage the chronic condition.

When appropriate, team care arrangements will be developed, providing up to five Medicare-subsidised referrals to specific allied health professionals per calendar year.
